State & Commission Forms
The following forms may assist you. Each should be completed, printed, and signed as applicable and mailed or emailed to the Cemetery Commission.

Monthly Reporting
The monthly reports are due the last day of the following month. For example: January 2024 reports are due February 29, 2024. A $25.00 late fee may be assessed for each late report for each month the report is late.
The cemetery may prepare these documents as PDF, XFA, or Excel forms and submit them digitally or printed, whichever is preferred.
These monthly reports consist of three documents:
1) Assessment Report*
2) Grave Space Report*

- NCCC Grave Space Report v 20 – XFA Version**
*Any fees owed must be sent via mailed check to NC Cemetery Commission – 1110 Navaho Dr. #107 Raleigh, NC 27609
** If you receive an error message, right-click and select “save link as” and rename the file prior to saving. Once saved, open the document from your computer files.
3) Pre-Need Liability Report – This report may be submitted in a format of your choosing as long as it contains all the same information.
Annual Reporting
Annual Reports for 2026 will be due to the Commission office by March 15, 2027. They may be submitted via mail or email. Mailed reports should be sent to: 1110 Navaho Dr. #107, Raleigh, NC 27609 — Email: reports@nccemetery.org
In the event mailed reports are received late, they will be considered received on the date they are postmarked. A $25 late fee will be assessed for late reports and will continue to accrue each month the report is late.

Only the annual report formats above will be accepted. Forms from previous years are considered obsolete and will be rejected. IF FORMS ARE ALTERED to remove or replace any data, forms will be rejected, and a late fee will be issued.
